Casenote reviewer for the 2016 RCPCH review
Consultant neonatologist and, since 2017, Medical Director at the Royal Free London NHS Foundation Trust. She carried out the casenote review that accompanied the 2016 RCPCH review of the Countess of Chester neonatal unit, examining the deaths and near-misses in the period. Her name was put forward as an independent person and, as she told the Inquiry, expressly not on behalf of the RCPCH.
Her review was a casenote review and could only work from what it was given. Asked whether such a review is 'a fairly superficial exercise', she answered: 'That is correct. It is only based on the information that's provided.' She was not told that a member of staff was suspected, and told the Inquiry that had the purpose been to exclude deliberate harm, a casenote review would not have been the right instrument.
